If It Is Not Written, It Did Not Happen

If it is not written, it did not happen. In Special Education, documentation is not paperwork for its own sake. It is a record of service, a mirror for inequities, and a roadmap for action. When we write clearly and consistently, we make it possible to see who is being served, who is being missed, and which practices move learning. When we fail to write, patterns of neglect hide in plain sight.

Start by setting a clean baseline for every student with an IEP. Establish attendance, present levels, current goal status, accommodations used, service minutes scheduled, and service minutes actually delivered. Capture family touchpoints. Note any barriers, such as transportation, assistive technology, or access to grade-level materials. This baseline is not a one-time event. It becomes the first line in a living story that you will update weekly.

Build a routine that fits the real life of a busy educator.
• Monday, log attendance, minutes scheduled, and minutes delivered.
• Tuesday, record family touchpoints and notes from co-teaching or consultations.
• Wednesday, verify accommodations used in general education settings.
• Thursday, check fidelity, list any missed minutes, and schedule make-ups.
• Friday, summarize goal progress with a short narrative and a simple rating.

Create a shared data dictionary so your team uses the same definitions. Delivered minutes means the time the student received direct or indirect services tied to an IEP goal. A family touchpoint means two-way contact, not a voicemail that never reached anyone. Goal progress note means evidence connected to the goal’s criteria, not a general feeling of improvement. Precision protects students.

Why this matters goes beyond compliance. In too many districts, service records read like case logs in the criminal justice system. The parallels are not accidental. When our language mirrors carceral systems, our practices drift toward surveillance instead of support. Clean, student-centered documentation interrupts that drift. It centers learning, not control. It creates a record that a student and family can hold with dignity.
